Welcome to the 2022 - 2023 ND Afterschool Program Self-Assessment & Action Plan Submission.
Helpful Tips:
Before submitting your completed Program Self-Assessment & Action Plan, make sure that you have worked with your Program Self-Assessment Review Members to complete the Self-Assessment & Action Plan for each of the four domains, included evidence of your ratings, and identified an action plan for your program site.
The Program Self-Assessment and Action Plan is most valuable when completed by multiple stakeholders. If you have multiple program sites, your Program Self-Assessment Team may look different for each site. This tool is an assessment, not an evaluation or report and there is no penalty for your ratings.
You are able to save and return to the form at a later time but will not be able to advance to the next page until all required fields are complete. Layout follows the ND Quality Standards for Out-of-School Time Programs: Health & Safety, Relationships & Interactions, Youth-Centered Program Design, and Leadership & Management.
Each indicator will ask for your rating (1, 2, 3, or 4) and evidence of your rating. How did you come to your rating? Depending on the indicator, this may be a policy, practice, or procedure you have in place. You do not need to upload your evidence but simply state what your rating is based on.
You can recieve an email of your submission by checking the box at the end of the form.
21CCLC Grant Requirements: If your program recieves 21st Century Community Learning Center funding, you are required to complete a Program Self-Assessment and Action Plan for each program site at least twice per program year. Deadline for Fall submission: October 10, 2022
Deadline for Spring submission: April 30, 2023
