SUMMER CAMP REGISTRATION FORM

This form is for Pre-K Counts registrants who are enrolling in Summer Camp when Pre-K Counts is not in session.

Summer Camp is available from Monday, June 1st through Friday, August 28th, 2020. Program hours 7:00 am to 6:00 pm, Monday through Friday.

Summer Camp Program is not free. Tuition rates are the same as the rates for our child care program. Tuition rates are available for your reference on the tuition page of our website and are as follows:

Five Days Per Week ... $210 per week
Four Days Per Week ... $176 per week
Three Days Per Week ... $138 per week
Two Days Per Week ... $96 per week

The Discovery Tree does accept child care subsidies, so if you are eligible please have your caseworker contact us with your child's approved start date. We cannot enroll your child for Summer Camp until we receive a verfication call from your caseworker.

After you fill out and submit this form, you will receive an invoice due for your child's first week of Summer Camp. That invoice must be paid in order for your child's enrollment to be complete.

If you want to pay the invoice online, it will give you an option to click and pay by entering your bank account information. We do not accept credit cards. You can also pay the Director in person by paper check or money order. No cash is accepted on the premises.

After the first week, you will be required to pay your summer camp tuition for the next week. Weekly payments are due on or before Monday of that week. All payments are due in advance of provided service. Families are not permitted to carry balances on
tuition accounts.

Tuition rates are based on enrollment, not based on attendance. No refunds will be issued for absenteeism.

Please be aware that lunch is not included as part of Summer Camp. The Discovery Tree provides breakfast, snacks, and beverages, but parents are required to send a packed lunch every day.

Contract for Child Care Services and Fee Agreement *

I confirm that I have had the opportunity to discuss the information contained herein with a representative of The Discovery Tree, that my questions have been answered fully and to my satisfaction and that I agree to abide by the conditions set forth herein as well as the policies contained herein by reference from the Parent Handbook

I understand that any requests for changes to the enrollment schedule listed herein must be made in writing and submitted to the Director in accordance with the Parent Handbook policies and will require payment of any additional security deposit, registration fees and/or tuition increase. The Discovery Tree reserves the right to deny any request for schedule change for any reason within its sole discretion.

I agree to compensate The Discovery Tree for the schedule of services stated above. I understand that Tuition is paid prior to the performance of child care services (accept on the scheduled start date, which is the free trial day). Tuition, in amounts stated on the Tuition Page of The Discovery Tree website, is due whether or not the child attends the program. Weekly payments are due on Monday of each week, and monthly payments are due on the first day of each month. The first payment is due on the day after the above stated start date.

The tuition represents the child's place in the program. There is no credit given for vacation or emergency closure days. If, pickup occurs after 6:00 pm, a late pick up fee of $1.00 per minute will be billed to the parents/guardian's account in accordance with the Late Pick-Up Policy outlined in the Parent Handbook. Late Pick-up fees are due on the day of lateness or on the following day.

The Contract for Child Care Services/Fee Agreement may be cancelled by The Discovery Tree at any time with or without notice, in its sole discretion. Any unused tuition paid by the parents/guardians will be refunded within 30 days of cancellation.

The Contract for Child Care Services/Fee Agreement may be cancelled by the parents/guardians with two weeks written notice. Written notice of cancellation must be submitted to the Director. Any unused tuition paid by the parents/guardians will be refunded within 30 days of cancellation. You will be notified in the event of an increase in tuition fees.

I understand that I will be entering into a Contract for Child Care Services/Fee Agreement with The Discovery Tree. The Discovery Tree agrees to provide child care services for my child in accordance with the mission, philosophy and policies outlined in the Parent Handbook.

I confirm that I have been given the opportunity to read the Parent Handbook, ask questions about it, understand the policies contained therein, and agree to abide by the policies. The Discovery Tree reserves the right to alter, amend, or otherwise modify these guidelines, in its sole discretion, without prior notice.

I agree to update the emergency contacts and parental consent information in my child's file whenever changes occur or every 6 months at a minimum. (§3270.124, §3280.124, §3290.124)

Emergency medical care and first aid authorization *

I authorize The Discovery Tree to obtain emergency transportation to and/or emergency medical care for my child at the nearest hospital's emergency room, or at the emergency room the EMS/Ambulance Service is required to transport patients to at the time of emergency.
