PVG ID Checking Form

Please use this form to submit the information required by BB Headquarters to process an online PVG application for a volunteer.

This form should be completed by the Identity Checker, and is used to collect information about the volunteer whose ID is being checked and that of the Identity Checker.

The Identity Checker will be asked to verify ID documents confirming the applicant's date of birth and current address. For more information about the routes and acceptable documents please visit boys-brigade.org.uk/pvg-scheme

Identity Documents

Original identity documents (not photocopies) must be produced that confirm the applicant's full name, date of birth and current address, in line with one of the following routes:

Route A: One form of photographic ID with applicant's date of birth and one document confirming current address (dated within 12 months, unless otherwise stated).

Route B: Where no photographic ID can be produced, one document confirming applicant's date of birth and two documents confirming current address (dated within 12 months, unless otherwise stated).
