Health Information

How would you describe your health? *
Are you currently under the care of a physician? *
I attest that I am aware of the physical requirements associated with Short Term Trip work (lengthy hours working, potentially daily travel on foot, weather considerations, vaccinations and responsibility to take medications as prescribed) and agree that I can meet these requirements. Should I be faced with physical limitations which will affect my ability to effectively minister on this assignment, I will make the Global Ministries Staff and the Team Leader aware immediately. *
