1. Please allow 30-60 minutes to complete this questionnaire.
2. Be thorough with this application and make sure you are comfortable with your estate planning decisions. You will be charged a fee for future changes to your Living Will.
3. Only complete the questions that are applicable to you. For example if you are not married, there will be several questions relating to children that may not be applicable to your situation.
4. If you are uncertian as to how to answer a question, feel free to contact your Alphamint Advisor, otherwise skip it and mention it during the follow up meeting where you and the Advisor will review the questionnaire together.
5. This questionnaire is not comprehensive and as such may not cover all aspects of estate planning, therefore it is your responsibility to discuss these additional items with your Advisor to ensure they are included in your will.
6. Please allow a few business days from the time of this submission for your Advisor to review it and follow up with a request to schedule a meeting to review the questionnaire.
7. This questionnaire is encrypted, however please ensure you are using a secure connection and not on public wifi.
8. Remember there are three reasons for estate planning. Our goal is to provide you the route to accomplishing these three goals and it is your reposnibility to be as thorough and focused with your goals. If you are still unsure who you want to inherit your estate, now may not be the time to complete this questionnaire.
