You will want to follow these easy instructions:
- Login to your Formsite account. **This will enable you to update your application from last year & save you time! If you are new to Maayan, create a Formsite account and login.
- Remember to click “Save Progress” at the bottom of every screen to save your updated information.
Families will need to submit immunization records for their child before they can attend.
